ESR test: normal range and what a high ESR means

Medically reviewed by Dr. N. S. Santosh Varma, MBBS, FIM, FIDM, FICCM · Reg. APMC/FMR 112686 · Updated the day of publishing

Quick answer

ESR (erythrocyte sedimentation rate) measures how quickly red blood cells settle in a tube in one hour. A high ESR is a general sign of inflammation, infection or some chronic conditions, but it does not say where or why. ESR rises with age and is usually higher in women. Doctors use it alongside symptoms and other tests.

Typical normal range

GroupCommonly used upper limit (mm/hr, Westergren)
Men under 50up to 15
Men over 50up to 20
Women under 50up to 20
Women over 50up to 30

Reference ranges differ slightly between labs, by age and by sex; always read the range printed on your own report and ask a doctor to interpret it.

Why ESR can be high

  • Infections
  • Inflammatory and autoimmune conditions such as rheumatoid arthritis
  • Anaemia and pregnancy
  • Kidney disease
  • Older age

A low ESR is rarely a concern on its own.

Preparation

  • No fasting needed for ESR alone
  • Tell the doctor about recent fever or infection

Frequently asked questions

What is a normal ESR for women?

Commonly up to about 20 mm/hr under 50 and up to 30 mm/hr over 50, but labs differ.

Is a high ESR serious?

Not always. It is a non-specific marker; a doctor looks for the cause with your symptoms and other tests.

Is ESR the same as CRP?

No. Both are markers of inflammation, but CRP changes faster. Doctors may order either or both.
